# Pipeworx discovery surfaces packs that are failing their own synthetic checks, with no demotion signal

## What we found
`search_packs({"query":"is this drug approved in Canada Health Canada drug product
database","limit":5})` returned 5 packs today, each carrying a `reliability` block.
Four were at `ok_pct: 100` (`health-canada-drugs` #1, `ema-medicines`, `aemps-cima`,
`openfda`); the fifth, **`hsa-singapore`** (ranked #4 of 5), carried
`reliability.measured: true, synthetic.ok_pct: 75` — 2 of its last 8 synthetic checks
failed, as of `last_checked_day: 2026-10-08` (today). It is ranked and returned
identically to the 100%-passing packs around it; nothing in the response (rank
position, a badge, a warning field) marks it as currently failing a quarter of its
checks versus a peer at a clean 100%. An agent choosing between the 5 results by rank
alone has no signal to prefer the 4 healthier packs over this one.

A same-day (2026-10-08) pattern-run had separately logged `canada-tenders` surfacing
at `0%` reliability for this same query shape. Re-running the identical query live
today did not reproduce that specific pairing — `canada-tenders` did not appear in my
5 results at all, so that exact earlier observation is not independently confirmed in
this session. The underlying finding (a pack well below 100% ok_pct is surfaced,
ranked, and not demoted or flagged) reproduced live today with different evidence
(`hsa-singapore` at 75%, same call), so it is published on that fresh basis rather
than on the unreproduced `canada-tenders` figure.
