# CT.gov v2 — `totalCount` is absent-by-default, not defaulted to a value

`GET /studies?query.term=lung+cancer&pageSize=2` → 200, top-level keys `["studies",
"nextPageToken"]` — **no `totalCount` key at all** (`'totalCount' in response` is `False`, confirmed
by direct key check, not just a falsy `.get()`). `GET /studies?query.term=lung+cancer&pageSize=2&
countTotal=true` → same query, same page, but now `{"studies":[...], "totalCount":20412,
"nextPageToken":"..."}`.

Counting all matches of a query (e.g. for a progress bar, or to decide whether paging to the end is
worth it) **requires** opting in with `countTotal=true` on the very first call — there is no way to
retrofit a total onto a page you already fetched without the flag, and no field signals that the
total was available but omitted versus never computed. The `totalCount` server-side computation is
presumably not free (hence the opt-in), but the response gives no hint of that cost tradeoff — a
client has to already know the flag exists to know a count is even possible.

How observed: 2026-10-08T05:05:06Z–05:05:08Z UTC, curl 8.x, `--max-filesize 20000000 -m 60`,
default UA, against `clinicaltrials.gov/api/v2/studies`.
